What do I need to bring to my child’s appointment?
Please bring your driver’s license or current ID and your child’s insurance card for proper registration and identification.
Who can take the child to the appointment?
Whenever possible, we prefer that the parent or primary caregiver attends the appointment. Grandparents, aunts, uncles, adult siblings and adult friends may also bring your child, but you must have granted previous permission. We will ask you to list people who are allowed to accompany your kid and have access to their health information on our initial demographics form.
